Valley View, BlueCross BlueShield and Wellness Inc. would like to help get you started!

Wellness Inc. will be coming to Valley View School District during the months of April and May to perform their Annual Wellness Screening on site.

Wellness screening is covered by the District's BCBS PPO Health Plan. If Valley View's BCBS PPO is your PRIMARY healthcare coverage your Wellness Screening will be covered at 100%. That's right, nothing to pay. Just bring your BCBS Health Member ID card for proof to the appointment and you will have no out of pocket cost.

If you are not covered under Valley View's health insurance, payment will be due at the time of service.

Wellness Inc. has provided a detailed report for you to review and to pass on to your health provider.

Also, with this report you can enter the information into BlueCross BlueShield's Health Risk Assessment (HRA).
