Health and Dental Open Enrollment for
Calendar Year 2011
Effective November 1 through November 24, 2010
Please Note: The Open Enrollment period has been shortened. This will ensure adequate time for all information to be processed by our NEW Third Party Administrator, Blue Cross Blue Shield, for a January 1, 2011 effective date.
If you would like to make any changes to your insurance coverage or would like to enroll in the Flexible Spending Accounts (FSA) please be sure to return the forms to the Administration Center, Attn: Lorrie Alsobrooks before November 24, 2010 11:59 pm. You can fax the forms to (815) 886-6386 or send them inter-school mail to the Administration Center, Attn: Lorrie Alsobrooks.

Your election to enroll is effective only for one plan year at a time. Therefore, each new plan year you must turn in an enrollment for to continue participation.
