Calendar Year 2012
Open Enrollment Period Nov. 1 – Nov. 30, 2011
If you would like to make any changes to your insurance coverage or would like to enroll in the Flexible Spending Accounts (FSA) please be sure to return the forms to the Administration Center, Attn: Lorrie Alsobrooks before November 30, 2011 11:59 pm. You can fax the forms to (815) 886-6386 or send them inter-school mail to the Administration Center, Attn: Lorrie Alsobrooks.
