- What is transition planning?
- Transition planning is a set of activities that helps students prepare to move from high school to employment, post-secondary education, adult living, and participating in the community.
- When does transition planning begin?
- In the state of Illinois, transition planning should begin at the age of 14, and transition planning should occur every year through 21 years of age.
- What is the transition page?
- It is a page on the IEP that helps coordinate transition activities. The transition page should include a vision statement indicating what the child would like to accomplish after high school in the areas of employment, post-secondary education, adult living, and community participation. The transition page should include activities that will help the student achieve their vision. It should also aid in the development of IEP goals, identification of course schedule, identification of placement, and identification of related services.
- When will the transition page be reviewed and updated?
- The transition page will be reviewed and updated during the annual review. The student, their guidance counselor, and their case manager will also look at the transition plan during registration for classes. Throughout the school year completed transition activities will also be listed.
- Who will participate in the transition planning process?
- The student, the student’s parents/guardians, an SRT leader, a special education teacher, a general education teacher and occasionally adult service agencies or other individuals knowledgeable about the transition process. All individuals participating will be listed on the conference notification letter.
- What are adult service agencies?
- Adult service agencies are community agencies and organizations that provide a range of services to individuals with disabilities after high school. Examples of such organizations are the Department of Rehabilitation Services, Cornerstone Services, and Easter Seals.
