Office Notes

Early Arrival Reminder: We would like to remind parents that students should not arrive to school earlier than 8:45 am for regular attendance in school and no earlier than 8:30 am if they are here for breakfast service. The exception to this, of course, would be those students enrolled in the Oasis Day Care program. Please be reminded that it is the parent’s responsibility to be sure that students are arriving to school at the appropriate times. We are not staffed to provide supervision of students earlier than the times mentioned above.

Cold Weather Reminder: All students coming to school should be dressed appropriately for the cold weather. Students will be waiting outside for school to begin as long as the temperature and wind chill are above 10 degrees. Also, we conduct outdoor recess by the same guideline, so gloves and hats are suggested on a daily basis.

Attendance Line Reminder: All student absences should be called in on our 24-hour absence line. The number is 815-886-7841. Please clearly provide student first and last name, grade, teacher and ALSO a reason for the absence. The office will contact you to verify the absence if you have not called to report your student absent.

Emergency Forms: It is imperative that you keep the information on the Emergency Release Forms current. We MUST be able to contact you if your student is ill, not picked up after school or there is some other kind of emergency. This form can be printed from this web site; or you can send a note with your student or call the office with any changes to your personal contact information and we will be happy to change the form already on file.

All Kindergarten and 5th grade students must have proof of a current physical and completed immunization records. 

EXCLUSION date for incomplete records is September 1, 2007.  Any student with incomplete health records will not be allowed to attend school after August 31, 2007.  All physicals must be completed on the Illinois Certificate of Child Health Examination form.  You can obtain this form by clicking on the Physical Exam Form above or visiting the school office. 

A sports physical is not acceptable as a school physical.  Appointment cards WILL NOT be accepted.  You may return the completed physical to the school nurse or at registration in August.
