Attention 9th Grade Students

All 9^th grade students must have proof of a 9^th grade physical and completed immunization records. Exclusion date for incomplete records is September 1, 2006. Any student with incomplete health records will not be allowed to attend school on or after September 1, 2006. Please bring the enclosed physical form and any immunizations records to your doctor for completion of the physical form. All physicals must be completed on the Illinois certificate of child health examination form. A sports physical is not acceptable as a school physical.

Appointment cards will not be accepted. You may return the completed physical to your school nurse, through mail in registration, or at walk in registration.
