Dear Parent or GuardianIf your child has a food allergy that requires a menu accommodation, your child's physician must complete and submit this form to your child's school nurse. The Nutrition Services Department will make breakfast and lunch accommodations for any child with a legal disability (refer to above document).Si su hijo/a es alérgico a un alimento que requiera una modificación al menú, el médico de su hijo/a debe llenar y someter esta forma a la enfermera de la escuela.
