Internal Revenue Service (IRS) Federal Form 1095-C
As part of the Affordable Care Act (ACA), employees, IMRF retires under the age of 65 and COBRA participants eligible for health insurance offered by the Valley View School District will be receiving a new IRS Federal Tax Form which will be distributed annually.
This January, in addition to your annual W-2, you should pay attention to a new document that we will send you called a "1095-C" form. As part of the healthcare reform, every individual must now show that they had health insurance in order to avoid a tax penalty under the individual mandate.
As your employer, we will file a copy of your 1095-C with the Internal Revenue Service (IRS) so they know whether or not you enrolled in our group benefit plan. We will send a copy to your current mailing address. Do not discard the 1095-C, you will need that information along with your W-2, when you complete your personal income tax returns for April.
If you enrolled in coverage for yourself as well as other members of your family, the 1095-C will show the months of the year that they were enrolled. If you have any questions, please contact the Employee Benefits Department at firstname.lastname@example.org
