Transition Planning Overview
A set of coordinated activities that involves students with disabilities, parents, schools, and community agencies in order to create a plan for employment, post-secondary education, community participation, and daily living skills.
Legal Definition
Transition services are a coordinated set of activities for a child with a disability that:- Is designed within a results-orientated process, that is focused on improving the academic and functional achievement of that child with a disability to facilitate movement from the school to post-school activities, including post-secondary education, vocational education, integrated employment, continuing and adult education, adult services, independent living or community participation
- Is based on the individual child’s needs, taking into account the child’s strengths, preferences, and interests
