    Please notify one of our school nurses if your child has any health concerns requiring support and resources in school. This can include asthma, severe  food and bee sting allergies, seizures, Diabetes or other acute or chronic health concerns.

    If your child requires any medication while at school, we ask that you comply with the District Medication guidelines.

    * IMPORTANT: For safety reasons, medication must be brought to school by parent/guardian and may not be sent in the child’s backpack.

     

    A child should remain home with the following symptoms   sick

    • a fever greater than 100 degrees until they are fever free for 24 hours without the use of fever reducing medication 
    • Symptoms that prevent participation in school such as: excessive and productive coughing, purulent nasal drainage, severe headache, earache sore throat and poor sleep the night before related to symptoms
    • vomiting or experiences diarrhea until free of these symptoms for 24 hours
    • Rashes that have not been checked by a doctor. If the student will be coming to school with a rash that the doctor has checked, a doctor’s note is required noting that the student is cleared to come to school
    • Red, pink, crusty, irritated eyes. If your student will be coming to school with red, pink, crusty, or irritated eyes that the doctor has checked, a doctor’s note is required noting that the student is cleared for school attendance   

    If your child stays home ill, please call the main office: (815) 886-7827.  It is District policy to provide a note from the doctor if your child is absent for 3 or more days. 

    If your child becomes sick while at school the school nurse will call you to pick up your child in a timely manner. If you are not able to pick up your child, you must make arrangements for an alternate emergency contact person to do so and inform the school of full name, relation and approximate time that they will be arriving.                     

    Un niño debe permanecer en casa con los siguientes síntomas: sick

    • Fiebre superior a 100 grados hasta que estén libres de fiebre durante 24 horas sin el uso de medicamentos reduce la fiebre
    • Síntomas que impiden la participación en la escuela, tales como: tos excesiva y productiva, drenaje nasal purulento, dolor de cabeza intenso, dolor de oído, dolor de garganta y falta de sueño la noche anterior relacionada con los síntomas.
    • vómitos o diarrea experiencias hasta que quede libre de estos síntomas durante 24 horas
    • erupciones que no han sido revisados por un médico. Si el estudiante llega a la escuela con una erupción que el médico ha comprobado, una nota del médico es necesario señalar que el estudiante está desactivada para venir a la escuela
    • rojo, rosa, crujiente, irritación de los ojos. Si el estudiante será venir a la escuela con rojo, rosa, costras o irritación en los ojos que el médico ha comprobado, una nota del médico es necesario señalar que el estudiante está desactivada para la asistencia a la escuela

    Si su niño permanece a casa enfermedad, llame por favor la oficina principal: (815) 886-7827. Es política del districto para proporcionar una nota del doctor si su niño está ausente por 3 o más días.

    Si su hijo se enferma en la escuela la enfermera de la escuela se pondrá en contacto con usted para recoger a su niño de manera oportuna. Si no son capaces de recoger a su niño, usted debe hacer los arreglos para una persona de contacto de emergencia alternativo para hacerlo e informar a la escuela de nombre completo, la relación y tiempo aproximado que se llegan.

    Vision and Hearing Screenings: 

    Vision and hearing screenings are provided annually to all Preschool students. These screenings are State mandated with the goal of finding children who may need a comprehensive Doctor’s eye and/or hearing evaluation. Early discovery and treatment can prevent or at least alleviate many problems that may affect educational performance. Parents may contact our School Nurse with any questions or concerns.

     

    IMPORTANT STATE OF ILLINOIS  and *DISTRICT FORMS REQUIRED TO START SCHOOLshf

    The state of Illinois requires physical examinations and required immunizations for every child when they start school. Students must have a physical examination and their immunizations completed before their first day of school or they will be excluded after a minimal timeframe until they have one. 

    Preschool Parents 

    • Physical examination recorded on the *State form completed within 1 year                                    
    • Required Immunizations
    • *District Student Health History Form

    Kindergarten Parents 

    • Physical examination recorded on the *State form completed within 1 year
    • Required Immunizations
    • Dental Examination performed by a licensed dentist recorded on the *State form by May 15. 
    • Vision Examination recorded on the *State form and required by October 15 
    • *District Student Health History Form

     

    *Exam forms are available in the health office at school or are available to download on this Student Health Forms link.
